Home Care Package Levels

Level 1: Basic Care Needs

Support provided: Suitable for those with basic care needs.

Services include: Assistance with daily activities, basic personal care, light housekeeping, and meal preparation.

Ideal for: Individuals who require minimal assistance to maintain independence at home.

Level 2: Low Care Needs

Support provided: For individuals with low-level care needs.

Services include: All Level 1 services plus more comprehensive personal care, regular health check-ups, and social support.

Ideal for: Those who need moderate assistance with daily tasks and personal care.

Level 3: Intermediate Care Needs

Support provided: Designed for individuals with intermediate care needs.

Services include: All Level 2 services plus more intensive personal care, medication management, transportation, and assistance with mobility.

Ideal for: Individuals who require more substantial support to manage their daily living activities.

Level 4: High Care Needs

Support provided: For individuals with high-level care needs.

Services include: All Level 3 services plus complex health care support, frequent health monitoring, specialised nursing care, and extensive personal care.

FAQ’s - Home Care Package

To apply for a Home Care Package, contact My Aged Care on 1800 200 422 to create a profile and arrange for an assessment. Once you are approved, you can choose My Guardian as your provider, and we will help you develop a personalised care plan to start services.

Home Care Packages include a range of services such as personal care, assistance with daily living activities, medication management, health services, meal preparation, housekeeping, transportation, and social engagement activities. The services are tailored to meet your specific needs.

The level of support is determined through a physical assessment from My Aged Care. Based on this assessment, you will be assigned one of four levels of care, ranging from basic support to high-level care, ensuring you receive the appropriate level of assistance for your needs.
