To begin your journey with aged care, the first step is to apply through My Aged Care, the Australian Government’s portal for aged care services. If you are aged 65 or above, you can apply for a Home Care Package. You can start your application online or by calling My Aged Care at 1800 200 422.
Once your application is submitted, it’s important to request an assessment. A representative from My Aged Care will evaluate your physical, emotional, and social needs. This assessment allows you to discuss your preferences and any specific support you may need for a tailored care plan that meets all your requirements.
After the assessment, you will receive a summary of the findings and suggestoions, including whether you’ve been approved for a package. We will work with you to create a personalised care plan that outlines the services you’ll receive, including daily assistance, medical support, social activities, and companionship.
